Condition guide

Fatty liver disease (MASLD)

Fatty liver is one of the most common liver conditions in the world. It usually causes no symptoms, which is why it is often found by chance. The important question is not only “is there fat?” but “is there scarring?”.

Key points

  • MASLD stands for metabolic dysfunction-associated steatotic liver disease. It is the new name for what was called NAFLD.
  • Most people with fatty liver do not develop serious liver damage, but a minority develop inflammation (MASH) and scarring (fibrosis), which can progress to cirrhosis over years.
  • Weight loss, healthy food and regular activity can reduce liver fat and, in many people, inflammation and early scarring.
  • Fatty liver also goes together with a higher risk of heart disease, so the heart and blood sugar need attention too.

Overview

Fatty liver means that more than about 5% of the liver’s weight is fat. In MASLD this happens together with at least one metabolic risk factor such as excess weight, type 2 diabetes or prediabetes, high blood pressure, high triglycerides or low HDL cholesterol.

In many people the fat is harmless for years. In some, the fat triggers inflammation and injury (metabolic dysfunction-associated steatohepatitis, MASH). Over time, repeated injury can lead to scar tissue (fibrosis) and, in a smaller group, cirrhosis. The amount of scarring, rather than the amount of fat, is the best guide to long-term risk.

If alcohol also plays a part, doctors may use the term MetALD or alcohol-related liver disease. This is why your doctor will ask you honestly and without judgement about alcohol.

Causes and risk factors

  • Excess body weight, especially fat around the waist
  • Type 2 diabetes or prediabetes (insulin resistance)
  • High blood pressure, high triglycerides, low HDL cholesterol
  • Sedentary lifestyle and a diet high in sugary drinks, refined carbohydrates and processed food
  • Family history and genetic factors, which is why some people with normal weight also develop it
  • Some medicines and conditions such as polycystic ovary syndrome or an under-active thyroid

Symptoms

  • Most people have no symptoms at all
  • Tiredness or a vague ache in the upper right abdomen in some people
  • Often discovered by chance on an ultrasound or in blood tests (raised ALT)

When to seek urgent medical care

  • Yellow eyes or skin, a swollen abdomen or legs, vomiting blood, black stools, or confusion can mean advanced liver disease and need urgent assessment

In an emergency go to the nearest emergency department or call Rescue 1122.

How it is assessed

  • Liver blood tests and an ultrasound scan. An ultrasound can show fat but cannot reliably show scarring
  • Simple blood-based scores (such as FIB-4) and, where available, elastography (for example FibroScan) to estimate stiffness of the liver. These help decide who needs closer follow-up
  • A check of blood sugar (including HbA1c), cholesterol, blood pressure and weight
  • Tests to exclude other causes of abnormal liver tests, such as hepatitis B and C, medicines, iron overload or autoimmune disease
  • A liver biopsy is needed only in selected people when the diagnosis is unclear

Treatment

Lifestyle is the foundation

  • Gradual weight loss if overweight. Losing about 5% of body weight can reduce liver fat, about 7 to 10% can improve inflammation, and more than 10% may improve scarring in some people. Crash diets and rapid weight loss are not recommended
  • At least 150 minutes a week of moderate activity such as brisk walking. Strength training helps too, even without weight loss
  • A Mediterranean-style pattern: vegetables, fruit, pulses, whole grains, nuts, olive or other unsaturated oils, fish. Cut down on sugary drinks, sweets, bakery items, fried and highly processed food
  • Avoid alcohol if you have fatty liver with significant scarring. Ask your doctor what is right for you if scarring is mild or absent

Treat the related conditions

  • Good control of diabetes, blood pressure and cholesterol. Statins are generally safe in people with fatty liver and protect the heart
  • Some diabetes and weight-loss medicines also help the liver in selected people. Newer medicines for people with significant scarring exist, but suitability and availability vary, so this is a specialist decision
  • People with cirrhosis need regular ultrasound screening for liver cancer and screening for varices

What you can do

  • Vaccination against hepatitis A and B if you are not immune
  • Avoid “liver detox” products and unlabelled herbal remedies. Some supplements can themselves harm the liver. Check with your doctor first
  • Treat sleep problems (such as sleep apnoea) and keep to regular sleep hours
  • Keep a simple record of weight, waist, blood sugar and liver tests to see progress over months, not days

Common questions

Can fatty liver be reversed?

In many people, early fatty liver improves substantially with sustained weight loss, healthy eating, exercise and good control of diabetes and cholesterol. If scarring has developed it may not fully reverse, but the same steps can still stop it from getting worse. Results differ from person to person.

I am thin. Can I still have fatty liver?

Yes. Some people with normal weight have fatty liver, often linked to insulin resistance, a diet high in sugar and refined food, low muscle mass or family history. They deserve the same assessment.

Do I need a biopsy?

Usually not. Blood scores and elastography are used first to estimate scarring. A biopsy is considered only if those tests are unclear or another liver disease is suspected.

Is a small amount of alcohol safe with fatty liver?

There is no completely safe amount for everyone. People with significant scarring should avoid alcohol. Please discuss your own situation with your doctor.
