Condition guide

Irritable bowel syndrome (IBS)

IBS is a common long-term condition of gut function. It can be troublesome, but it does not damage the bowel and does not turn into bowel cancer.

Key points

  • IBS is diagnosed mostly from your symptom pattern, with a few simple tests to rule out other conditions.
  • Triggers vary: certain foods, irregular meals, stress and poor sleep are common.
  • Soluble fibre, regular routine, exercise, stress management and targeted medicines help many people.
  • Blood in the stool, weight loss, anaemia, or symptoms waking you at night are not typical of IBS and need tests.

Overview

IBS involves a sensitive gut and changes in how the gut and brain communicate. Doctors often use the Rome IV criteria: recurrent abdominal pain on average at least one day a week for three months, linked to bowel movements and a change in how often or how loose or hard the stool is.

IBS is grouped by the main stool pattern: with constipation (IBS-C), with diarrhoea (IBS-D) or mixed (IBS-M). A gut infection can trigger IBS in some people. Symptoms come and go, and many people find one or two triggers that help them manage it.

Causes and risk factors

  • Gut sensitivity and altered gut motility
  • Previous gastroenteritis (infection)
  • Stress, anxiety, low mood and poor sleep
  • Certain foods (for example high-FODMAP foods, caffeine, very fatty meals) in some people

Symptoms

  • Cramping abdominal pain, often eased or changed by a bowel movement
  • Bloating and gas
  • Constipation, diarrhoea, or alternating between the two
  • Symptoms that flare with stress, certain foods or irregular meals

When to seek urgent medical care

  • Blood in the stool, unintended weight loss or anaemia
  • Pain or diarrhoea that wakes you from sleep
  • A new change in bowel habit after about age 50, or a family history of bowel cancer, coeliac disease or inflammatory bowel disease

In an emergency go to the nearest emergency department or call Rescue 1122.

How it is assessed

  • Diagnosis rests mainly on your symptom pattern and examination
  • Simple tests (blood count, coeliac blood test, CRP, stool calprotectin when diarrhoea is prominent) are often done to rule out other conditions
  • Colonoscopy is considered when there are warning features, or in older patients with new symptoms

Treatment

Diet and lifestyle

  • Regular meals, enough fibre of the right type (soluble fibre such as ispaghula often suits better than coarse bran), physical activity, good sleep
  • A guided trial of a low-FODMAP diet in selected patients, ideally with a dietitian, as it is restrictive and should be re-expanded after testing
  • Stress management, and for some people gut-directed therapies such as cognitive behavioural therapy or hypnotherapy

Medicines

  • Chosen for the main symptom: antispasmodics or peppermint oil for pain, loperamide or other medicines for diarrhoea, osmotic laxatives for constipation, and low-dose neuromodulators for persistent pain in some patients
  • Some medicines are used only for short periods. Do not use long-term laxatives or antidiarrhoeals without review

What you can do

  • Eat at regular times and chew slowly
  • Keep a simple food and symptom diary for 2 to 4 weeks to spot your triggers
  • Increase fibre gradually to avoid gas
  • Exercise, sleep and relaxation help IBS more than most people expect

Common questions

Can IBS turn into cancer or colitis?

No. IBS does not cause bowel cancer or inflammatory bowel disease. But the symptoms overlap with those conditions, which is why warning features are checked before the diagnosis is accepted.

Do I need a colonoscopy?

Not for everyone. It is recommended if you have warning features such as blood in the stool, anaemia, weight loss, a strong family history, or new symptoms at an older age.

Is IBS “all in the mind”?

No. IBS is real and involves the gut and the gut-brain connection. Stress can worsen it, but stress is not the only cause.
